The Tract CONNECT (Southern New England) mapping layer contains essential baseline data collected and displayed by US CENSUS Tracts relating to population in the years between 2020 and 2029 which support neighborhood organizations and state partners to CONNECT to data produced in reference to 2000 US CENSUS Tracts to build awareness, produce reports generate actionable insights, and address Social Determinants of Health factors that contribute to health variations.
