Description: <div>Background:</div><ul><li>FEMA'S Damage Assessment form V2.7 for Individual Assistance (IA) brings standardization to the collection of incident reported damages in impacted areas to determine the extent of the disaster.</li><li>Originally created by Jason Ray for the Florida State Emergency Response Team (SERT)</li><li>This was originally and updated release to FEMA Recovery's first operationally capable Survey 123 form. In this version Individual Assistance and Public Assistance Forms are separate in order to focus collection by program area and expedite the entry of damage assessments.</li><li>Examples of fields collected by the form for damages to individual facilities includes location information, cause of damage, ownership, insurance, type of dwelling, immediate needs, functionality of utilities, and the level of damage assessed according to the 2017 FEMA Damage Assessment manual criteria.</li></ul><div>This point layer has been tailored for Walton County Emergency Management. The locations are within Walton County, Florida only. </div>
